WELCOME NEW CLIENTS
Please complete all forms in its entirety before your initial session. Upon booking your first paid service with Sage Integrative Wellness, LLC an email will be sent inviting you to click the Client Portal Button/link to take you to Client Portal login page to create a client account and submit required forms and new client information within 6 months to 1 year. This will give me an idea of what state your body is currently in. You may also access the Client Portal login page on our website from the Client Portal tab to login into client portal once a client account has been created by invite.
A minimum of 24 hours is required for any cancellation of appointments. We do not provide refunds or returns. There is a cancellation fee of your scheduled appointment should we not receive your proper notice.
Unfortunately we do not accept any insurance.
